A doctor-led, medical approach to midlife weight and metabolic health. Personalised assessment and supervised care for women with Dr Tripti Gupta
When your body starts holding on to weight no matter what you do, the answer isn't trying harder — it's understanding why. A doctor-led, medical approach to weight and metabolic health for women.
You're doing everything you used to. So why isn't it working?
For many women, weight becomes harder to manage in midlife — particularly around the middle — even when nothing about their effort has changed. The diets that once worked don't anymore. The advice to simply "eat less and move more" starts to feel both exhausting and beside the point.
It's not that you've stopped trying. It's that your body's hormonal and metabolic landscape has shifted — and that needs to be understood, not ignored.
Weight in midlife is common — and worth taking seriously
Around 6 in 10 women in England are overweight or living with obesity.
The likelihood of carrying excess weight rises through midlife.
Where weight sits matters — fat around the abdomen is linked to higher risk of type 2 diabetes and heart disease.
Among adults living with obesity, the rate of diabetes is around three times higher than among those at a healthy weight
What's actually driving the change
Weight in midlife is rarely about a single thing. Several shifts tend to happen at once:
Changing hormones — falling oestrogen is associated with a shift in where the body stores fat, with more settling around the abdomen.
Insulin and metabolism — insulin sensitivity can decline with age, making weight harder to manage and energy less stable.
Muscle and metabolic rate — muscle mass naturally falls over time, which can lower the rate at which the body uses energy.
Sleep and stress — disrupted sleep and raised stress hormones influence appetite, cravings and how the body stores fat.
The wider picture — thyroid function, medications and other health factors can all play a part.
These factors interact — which is why a plan that addresses only one of them, or only calories, so often falls short.
A medical approach — not another diet
Primewellbeing takes weight seriously as a clinical issue. Rather than handing you a meal plan and a target, Dr Gupta begins by understanding the full picture — your history, your hormones, your metabolism, your sleep, stress and lifestyle — and identifying what's actually making weight harder to manage for you.
From there, you'll have a personalised, medically supervised plan grounded in lifestyle medicine and reviewed regularly as you progress. It's a doctor-led programme built around your health and sustained over time — not a quick fix, and not a one-size-fits-all protocol. Where any prescription is clinically appropriate, it is provided as part of this structured, closely monitored programme
Looking beneath the surface
Because weight is rarely the whole story, a proper assessment looks at the factors underneath it. Depending on what's right for you, this may include reviewing:
Metabolic markers such as glucose and insulin regulation
Thyroid function
Cholesterol and cardiovascular markers
Relevant hormone levels
Iron, vitamin and inflammatory markers
Results are always interpreted in the context of your symptoms, history and goals — so your plan is based on what's really going on, not guesswork. (Blood testing is arranged where clinically useful.)
What care with us looks like
This is ongoing, medically supervised care — not a single appointment. Your programme typically includes:
A thorough initial assessment of the metabolic, hormonal and lifestyle factors at play
A personalised plan grounded in evidence-based lifestyle medicine
Regular medical review to track your progress, health markers and wellbeing
Adjustment over time, so your care responds to how your body actually changes
The goal isn't a number on a scale by Friday. It's better metabolic health, more energy, and changes you can sustain — overseen by a doctor, every step of the way.
What to expect
Initial consultation (45 min) — a thorough "360° life audit": your story, symptoms, medical and family history and goals, including any recommendations for blood tests if needed. Your plan may include lifestyle guidance, and treatment or prescriptions where clinically appropriate. Includes one month of email support.
Your personalized plan — lifestyle guidance, and treatment and prescriptions where clinically appropriate, with anything prescribed dispensed through a registered pharmacy. All tailored to the factors driving your weight and metabolic health.
Ongoing reviews (30 min) — usually 4–10 weeks after starting, then every 3–6 months, to monitor progress, health markers and wellbeing, and refine your plan.

Common questions
-
This is medical care led by a doctor. Rather than a generic plan, we assess the hormonal, metabolic and lifestyle factors specific to you, and your care is medically supervised and reviewed over time.
-
No. Lifestyle is part of the picture, but we start by understanding why weight has become harder to manage for you — and build a plan around that, not around willpower.
-
Often, yes — metabolic and hormonal markers help us understand what's driving your symptoms — but we'll advise what's genuinely useful for you rather than testing for the sake of it.
-
Very often these are exactly the women we help. A medical assessment can identify factors that previous approaches never addressed.
